Immunization Requirements

Agnes Scott College intends to provide a safe and healthy environment for our students.  Therefore, all students are required to satisfy the Health Entrance Requirements (complete list of requirements below).  

Beginning Summer 2024 Submission Process

To upload your required documents:

  • Follow this link to Med+Proctor: http://secure.medproctor.com/Account/Signin
  • Enter your Agnes Scott email address to register.
  • Create your password (we suggest using your Agnes Scott campus password to make it easier to remember).
  • Upload your required documents.
  • The Med+Proctor Portal will have the same Entrance Health Requirements Packet that is available on the right side of this webpage (the yellow rectangle).

Undergraduate Requirements

Forms for incoming students are required before arrival on campus unless otherwise communicated.

Students must have the medical exam form (found in the Entrance Health Requirements Packet in the yellow rectangle on the right side of this webpage; it is also available in the Med+Proctor portal) OR one provided by a doctor or medical provider indicating a physical exam was conducted within 1 year of arrival to campus. 

Students who are under 18 years old when they arrive on campus must complete a parental consent form.

Students will no longer be required to submit proof of COVID-19 vaccination to the college. We strongly encourage our campus community to maintain their COVID vaccination status. This is an important step to the wellness and care for our students.

All students born on or after January 1, 1957, must meet this requirement, either by having been vaccinated against the three diseases (either as the combined vaccine MMR or individual vaccinations against the three diseases) or showing laboratory evidence of immunity to all three diseases.

EITHER:

Measles, Mumps, Rubella (MMR)

All students born on or after January 1, 1957 must meet this requirement, either by (a) having been vaccinated against the three diseases with the combined vaccine MMR, (b) individual vaccinations against each disease, or (c) providing laboratory evidence of immunity to all three diseases: 

(a) Combined Measles, Mumps, Rubella (MMR): 

  • Dose 1: On or after 12 months of age (provide month and year), and 
  • Dose 2: At least one month after first dose (provide month and year)

OR

(b) Individual Vaccinations against all three of the following:

  • Measles (Rubeola, Red Measles or Ten-Day Measles) - two doses of vaccine
    • Dose 1: On or after 12 months of age (provide month and year), and
    • Dose 2: At least one month after first dose (provide month and year)
  • Mumps - two doses of vaccine
    • Dose 1: On or after 12 months of age (provide month and year), and
    • Dose 2: At least one month after first dose (provide month and year)
  • Rubella (German Measles or Three-Day Measles) - two doses of vaccine
    • Dose 1: On or after 12 months of age (provide month and year), and
    • Dose 2: At least one month after first dose (provide month and year)

OR

(c) Lab results showing a Positive Antibody Titer for all three diseases listed above: Measles, Mumps, and Rubella 

 

All students must have the basic primary series of three doses of diphtheria and tetanus toxoid (DT or Td). These are usually given with the Pertussis vaccine (DPT) in infancy. In addition, all students must have a Td or Tdap booster within the past 10 years.

Students will no longer be required to show proof of immunization against Hepatitis A. Students who are in the middle of the Hepatitis A series are encouraged to complete the series but will not need to submit proof of immunization.

All students must have a series of three hepatitis B vaccinations or positive antibody titer (include a copy of lab results).

All students must have a history of Varicella by providing a positive Varicella antibody titer with a copy of lab results, or documentation of two doses of vaccine given at least one month apart. 

All students under 22 years old must have a dose on or after their 16th birthday.

The TB Questionnaire Form must be filled out, and any further testing indicated by the student's response(s) must be conducted. 

International students must have their Tuberculin Skin Test (TST) placed within the United States OR have an Interferon Gamma Release Assays (IGRAs) drawn within one year of orientation. If either test is positive, the student must have a chest x-ray performed to rule out active tuberculosis.

Graduate & Post - Baccalaureate Requirements

Students will no longer be required to submit proof of COVID-19 vaccination to the college. We strongly encourage our campus community to maintain their COVID vaccination status. This is an important step to the wellness and care for our students.
